This document is intended to satisfy the requirements of the Plan Document and Summary Plan Description (SPD) as specified in the Employee Retirement Income Security Act (ERISA). You can determine your rights under this Plan by consulting this SPD.
The Fund has made every effort to make this Plan Document and SPD as accurate as possible.
This Plan Document and SPD was published in July 2005 and is effective for benefits on and after August 15, 2005. It replaces all Healthlines, SPDs and plan documents previously published or made available.
The Board of Trustees expects to continue this benefits Plan indefinitely, but reserves the right to change or terminate the Plan at any time. If the Plan is terminated, benefits will be settled according to provisions of the Plan.
